To enter you must utilize the on-line registration form and provide complete and
accurate information. Limit one registration per computer/email address. Multiple,
incomplete or inaccurate registrations will be voided. Over the course of the promotion
there may be different methods of online entry.

provided (http://www.lifehome.com/drawing.htm). By its use
the registrant certifies he/she is at least 18 years of age and a United States resident.

General Conditions:
Use of automated entry devices or programs is prohibited. The prize will be awarded and
winners will be notified by e-mail or regular mail. Unclaimed prizes will not be awarded.
All registrants must be 18 years of age or older. Prizes are nontransferable and no
substitutions are allowed except by sponsor who reserves the right to substitute a prize
of equal or greater value if advertised prize is unavailable. By entering any contest,
each participant releases the Sponsor and its agents from all responsibility or liability
for damages, losses or injury resulting from acceptance or use of any prize. By entering
any sweepstakes, entrant recognizes and agrees that the registration information provided
by entrant may be made available to Life@Home, Inc., use of such information will be
governed by Life@Home, Inc. privacy policy. Taxes, if any, related to the prizes are the
responsibility of the individual winner. All prize winners may be required to verify
address or execute and return an affidavit of eligibility and liability/publicity release
within 14 days of notification attempt or the prize may be forfeited and alternate winners
selected. Acceptance of prize constitutes permission to use winner's names and likenesses
for promotional purposes without further compensation except where prohibited by law.
Life@Home,Inc. assume no responsibility for any injury or damage to participants or to any
other persons computer relating to or resulting from entering or downloading materials or
software in connection with this sweepstakes. Life@Home, Inc. is not responsible for
technical, hardware, software or telephone malfunctions of any kind, lost or unavailable
network connections, or failed, incorrect, incomplete, inaccurate, garbled or delayed
electronic communications caused by the user or by any of the equipment or programming
associated with or utilized in this sweepstakes or by any human error which may occur in
the processing of the entries in these sweepstakes. Life@Home, Inc. reserves the right, at
its sole discretion, to cancel, terminate, modify or suspend the online portion of the
promotion if these sweepstakes are not capable of running as planned, including, but not
limited to, infection by computer virus, tampering, unauthorized intervention, fraud,
technical failures or any other problems beyond the control of Life@Home, Inc.
